This report is to be completed by recipients of restricted grants from a donor advised fund or designated fund only. That means if your organization received a grant for a specific project or program, this form must be completed within 11 months of the award.
Follow-up reports for grants awarded through our competitive grant application process must be completed in the same online grant portal from which you submitted the grant.
Why is this grant report needed? The grant you received was only possible because of generous donors. We must share with them the impact of their giving to help ensure future gifts that could potentially benefit the good work of your organization again. Your responses will be shared with the donor(s) whose gift made your grant possible. Thank you for your help.
